Here it is. The Simplex Accident Reporter Kit. Used by the police but also very popular with insurance companies for accident claims. I first came across them when I was told a story about a driver at Wild's Brothers who was filling out a report for the insurance company and was heard to say "is there a stamp for a steam roller?".
